Designing appointment systems that reduce waiting and no-shows

Buffer time, realistic slot lengths and simple reminders outperform overbooking strategies.

Waiting time in clinics is usually a scheduling design problem rather than a capacity problem. Slots that are shorter than the real average consultation guarantee accumulating delay.

Measure the actual distribution of consultation length before setting slot durations, and build in buffer capacity for the unavoidable overruns.

No-shows fall substantially with two simple measures: a reminder that allows one-tap cancellation, and short intervals between booking and appointment.

Offer a small number of same-week urgent slots held back until forty-eight hours before. This absorbs demand that would otherwise arrive unannounced.

Publish your own metrics internally: average wait, overrun, and no-show rate by clinic. Teams improve what they can see.
