Coordinating care for older relatives across two continents

Polypharmacy review, a named local contact and clear escalation rules prevent most avoidable crises.

Older patients cared for across borders face two recurring risks: uncoordinated medication and delayed response to deterioration.

A structured medication review at least annually is the highest-value intervention. Prescriptions accumulate from multiple clinicians and are rarely stopped by anyone.

Identify a named local contact — a relative, neighbour or carer — who can attend appointments and report changes reliably.

Agree escalation rules in advance: which symptoms warrant same-day assessment, which hospital to attend, and who to inform.

Discuss goals of care while the patient is well. These conversations are far harder, and far less faithful to the patient's wishes, during a crisis.
