Blood pressure: the measurement most people get wrong

Home measurement technique changes readings more than most patients expect, and treatment decisions follow.
Hypertension is the leading modifiable cause of stroke, heart failure and kidney disease, and most decisions about it rest on readings taken by patients themselves.
Technique matters. Sit quietly for five minutes, back supported, feet flat, arm at heart level, cuff on bare skin and correctly sized. Take two readings a minute apart, morning and evening, for seven days.
Discard the first day and average the rest. A single high reading in a clinic is a poor basis for starting lifelong medication; a week of home readings is a good one.
Non-pharmacological measures are genuinely effective: reduced salt, weight loss, regular activity, moderated alcohol and better sleep can each lower systolic pressure by several millimetres of mercury.
If treatment is needed, adherence is the deciding factor. Simplified regimens and single-pill combinations consistently outperform complex schedules.
