Durable partnerships transfer systems and training rather than equipment alone.

Hospital partnerships fail in predictable ways: donated equipment without maintenance contracts, training without follow-up, and projects designed around the visiting institution's calendar.

Durable models share three features: local ownership of objectives, transfer of systems rather than objects, and funding that covers the recurring costs of operation.

Twinning arrangements with regular case discussion sustain skills better than intensive short visits, and they cost less.

Governance should be explicit from the start: who decides, how disputes are handled, how data is shared, and how the partnership ends.

Evaluate against local outcomes agreed at the outset, not against activity counts that flatter both sides.
